Integrating FQHC Health Care Services with
Permanent Supportive Housing in Los Angeles

UHHP, in collaboration with the Corporation for Supportive Housing, is delighted to announce the publication and availability to download “Integrating FQHC Health Care Services with Permanent Supportive Housing in Los Angeles.”  In addition to the pdf version that can be downloaded here, printed copies of the guide can be ordered on the CSH web site (www.csh.org).

The guide focuses on four areas:

  • Setting Foundations: Understanding Each Other’s Business
  • Three Program Models In Los Angeles
  • Developing Service Partnerships
  • "Operationalizing" It All

The guide was developed specifically for Los Angeles housing, health care, and other service providers.  It is based on leading practices in partnering these organizations to ensure the high level of services required for permanent supportive housing.  A training program covering the information in the guide is underdevelopment.  UHHP is collecting an interest list for the training on our web site (www.uhhpla.org). Please see below for more information.

We’d love to have your feedback on the guide.  Drop a note to info@uhhpla.org with your questions, comments, and suggestions.
